SFDTI LDPP will incentivize increased referrals between the primary care and dental clinics in local Federally Qualified Health Centers (FQHCs). The goal is increase the proportion of dual primary-care and dental users. Over the 4 years of the project, SF DTI LDPP will provide financial incentives to clinic sites to increase the proportion of Medi-Cal beneficiaries age 0-5 with both a primary care and dental visit within the same 12 month period.
8 FQHCs will participate in this pilot of the SF DTI LDPP. 4 of the 8 FQHCs are San Francisco Health Network clinics, and the remaining 4 are community based health centers.
SF DTI LDPP staff will provide support and technical assistance and work individually with each clinic for unique strategies to increase the proportion of dual users.
